Pre-Course Assessments

Two quick, standardized assessments. They give Dr. Rikki Permenter a baseline so she can walk with you well — and let us track how the forge actually changes men.

Complete this four times: baseline (before you arrive), immediately after the course, 2 weeks later, and 6 months later. Just pick the right timepoint below each time. Takes about 3 minutes.

1 · Personal Wellbeing (ONS4)

0 = not at all · 10 = completely. Answer for how you feel overall.

1. Overall, how satisfied are you with your life nowadays?

2. Overall, to what extent do you feel the things you do in your life are worthwhile?

3. Overall, how happy did you feel yesterday?

4. Overall, how anxious did you feel yesterday? (0 = not at all anxious, 10 = completely anxious)

2 · GAD-7

Over the last two weeks, how often bothered by… (0 = not at all, 1 = several days, 2 = more than half the days, 3 = nearly every day)

1. Feeling nervous, anxious, or on edge

2. Not being able to stop or control worrying

3. Worrying too much about different things

4. Trouble relaxing

5. Being so restless that it is hard to sit still

6. Becoming easily annoyed or irritable

7. Feeling afraid as if something awful might happen
